In order to fill out the form, you may need to download the file, enable editing, and/or change the setting
from "viewing" to "allow editing".
If you are selected as a vendor, you will receive a confirmation email with information
about how to submit payment.
Please use the .PDF version of the application if you wish to fill out the application by hand or mail it through the US postal service. Please read the instructions carefully and fill out the entire application, including photos, item descriptions, etc. All applicants, even returning vendors, must submit a complete application to be considered.
